Stage three kidney disease, also known as moderate chronic kidney disease (CKD), is characterized by a glomerular filtration rate (GFR) between 30-59 ml/min/1.73m². The diagnosis of stage 3 CKD is typically confirmed through blood tests to measure creatinine levels and estimate the GFR.

Clinical manifestations of stage 3 kidney disease may include fatigue, swelling in the legs or face, changes in urination patterns, nausea, vomiting, and itching. Patients may also experience hypertension, anemia, and electrolyte imbalances.

Management of stage 3 kidney disease focuses on slowing down the progression of the disease and managing symptoms. This includes controlling blood pressure with medications such as ACE inhibitors or ARBs, managing blood sugar levels in diabetic patients, monitoring and treating anemia with iron supplements or erythropoietin-stimulating agents, and addressing any nutritional deficiencies.

As a nurse caring for a patient with stage 3 kidney disease, it is important to focus on providing holistic care that addresses both physical and emotional needs. Some key nursing interventions include:

1. Monitoring vital signs regularly to assess for signs of fluid overload or electrolyte imbalances.

2. Educating the patient about their condition, including dietary restrictions (low sodium, low potassium), medication management, and lifestyle modifications (smoking cessation, regular exercise).

3. Encouraging adherence to prescribed medications and follow-up appointments with healthcare providers.

4. Providing emotional support and counseling to help the patient cope with the challenges of living with a chronic illness.

5. Collaborating with other members of the healthcare team, such as dietitians and social workers, to ensure comprehensive care for the patient.

In conclusion, stage three kidney disease requires careful management to prevent further decline in kidney function and manage associated complications. Nurses play a crucial role in providing education, support, and coordination of care to help patients maintain their quality of life despite their diagnosis.
