MAPP: Mobilizing for Action through Planning and Partnerships - a model for improving community health systems. This model focuses on developing partnerships and strategic action. The Final Project for this course builds on the learning and skills that you have developed along the way. These include critical thinking skills as well as systems thinking. You previously submitted your community health topic selection for this project in Module 3 and should have received approval by the instructor. Now you will utilize the guiding principles of the MAPP model and present a well-developed and supported 7-10 page paper that outlines the use of the six (6) MAPP phases for exploring a community health problem in your local community. This may be a problem that has been long-standing but has not been addressed effectively or it may be an issue that has been newly identified. You may address a problem in your town/city or in your county. You will use the elements of the MAPP model to explore ways to understand the underlying structure of the community health issue you have identified and develop a specific plan for action that improves the health and quality of life in your community.
