A 65-year-old woman with worsening dyspnea, cough, and sputum production over several days is admitted to the medical unit with COPD exacerbation. She has a history of emphysema from cigarette smoking. She has been taking tiotropium and Advair routinely for years. She is allergic to penicillin. On admission, the following medications are ordered: azithromycin 500 mg PO daily prednisone 40 mg PO daily x 5 days albuterol 2.5 mg by nebulizer q 4 h ipratropium 500 mcg by nebulizer q 4h albuterol 2.5 mg by nebulizer q 2 h prn oxygen by nasal cannula to maintain oxygen saturation of 88-92%.
