The patient is a 13-year-old African American female, seen with her mother, who reports persistent sadness, worthlessness, and anxiety for five years, worsening in the last two. She experiences fatigue, concentration difficulties, irritability, social withdrawal, and hopelessness, losing interest in activities like band practice, which she quit six months ago. Her worries about school are significant, with academic performance declining, resulting in being held back a year. Stress from school disrupts her sleep. Her teachers first noticed her struggles with focus, and her mother observed increased frustration and isolation at home. A major emotional trauma includes the death of her father in a car accident when she was eight, leaving her with a heightened fear of losing her mother, who has a history of alcohol use disorder that worsened after her husband's death. The patient feels pressured to care for her mother and shares a family history of depression. While denies current suicidal thoughts or self-harm behaviors, she occasionally feels that her life wouldn't matter. She does not use drugs or supplements.
