18-month-old with cough that started five days ago and worsened with fever, runny nose, and difficulty breathing, which started two days ago. Cough is productive with small amounts of yellow sputum, is exacerbated by physical activities, and occasionally disrupts his sleep. Temperature was 102 and Tylenol given. His breathing improves when sitting up and worsens when lying down. He has decreased appetite but normal fluid intake with 4-5 diaper changes daily. Mother reports sick contacts at daycare and exposure to secondhand smoke at home. Physical examination reveals patient is febrile with a temp of 102; skin is very warm and tenting, Turbinates are boggy and gray with thick, tenacious yellow drainage bilaterally. He is tachypneic with RR 40 and effort labored, has intercostal retractions, and nasal flaring. Fine crackles auscultated bilaterally. Palatine tonsils grade 2+, throat is erythematous. Immunizations up -to-date and his last well-child visit done 3 months ago WNL
