A 35-year-old male patient arrives directly at the hospital after a train tanker carrying pressurized liquid chlorine is breached during a collision. The tanker released a cloud of yellow-green gas that killed 6 and sickened more than 100 people in proximity to the crash. This patient is the first of many arriving at area hospitals. The patient has red, irritated eyes and shortness of breath. A section of his sleeve appears damp, and the underlying skin is visibly irritated. The emergency department (ED) nurse immediately moves to conduct a focused physical assessment.

Identify the clinical problem(s) in the case study. The patient attempts to help with decontamination by pulling his shirt over his head. Why does the nurse stop him?
