Question

My preceptor plays great role for me to learn during my practicum rotations. My mentor is very knowledgeable and very dedicated to guide me so I can acquire the knowledge needed. I do have opportunity to have hands on to participate in all meetings, workshops when I am available in the site. These collaborations with other healthcare providers and interdisciplinary teams enhance my understanding on the importance of are coordination's, team work, leadership strategies and patient advocacy. From the nature of patients seen in the practicum site which is the primary care provider ( PcP) most patients are seen with multiple chronic disease and are non-adherent to treatment plans. As a results of that they suffer from compilations and compromise their overall wellbeing. This drives me to focus on identifying the root causes of the problems by focus of patient entered care and empowerments and the adoptions and utilization of EBP strategies that will benefit both patients and practicum site.
