You are required to complete an incident report (actual or simulated) to provide evidence of your skills, knowledge and ability to deal with accidents and incidents appropriately and report to the supervising allied health professional.

If it is to be completed in a simulation, the following scenario may be used.

You are about to start a clients therapy and the supervising allied health professional is not currently in attendance. You have been asked by the practice manager to start the therapy as the supervisor is running late. You identify this to the client and they have given you their consent to start. You access and follow the required organisation policy and procedures in relation to infection control, however you incorrectly lift a piece of equipment and twist you back causing an injury.
