Boyd is now being evaluated in the pediatrician office. He is accompanied by his mother who describes that his condition is worsening. She shares that "the upper right thigh spot is growing, and his right knee is red." She also describes a new "spot" on the left lower leg. Boyd's mother shares that she has been searching online to learn more about Boyd's condition and is concerned that the areas "look just like spider bites."
Upon inspection the legs look as follows:
His vitals are 98.7 (oral, °F), 90 BPM, 16 respirations, 92/58 BP, and 98% on room air. Boyd reports intermittent pain to the right knee that is at a level 3 at worse (on a 0-10 scale). He describes that he has chills at times, and his mother reports that she has been giving him acetaminophen and ibuprofen for his pain. He states that "when I shower stuff drains out of a few of the spots." When asked to describe it he says it is "brown and nasty." The pediatrician incises the area to the thigh and lower leg and a culture is obtained. Boyd is sent home with instructions to continue keeping the areas clean and continue to apply the topical ointments and a new prescription for minocycline 75 mg by mouth twice a day.
