A 27 year old woman presents with irritability, sleep disturbance, poor self esteem, poor energy and anhedonia approximately 7 days before the onset of her menstrual cycle for the past several years. She has tried 1200mg of calcium supplementation daily, and this helped somewhat, but her symptoms did not resolve. She denies that any of these symptoms are a problem the rest of the time. She is willing to try an antidepressant medication, but is concerned about side effects. Which would be your first line medication regimen for this patient?
