In many scenarios, a lack of interprofessional collaboration has impacted patient care, which may harm or delay care. One scenario I experienced a lot is the ineffective communication between physicians, nurses, and social workers or case managers. The patient is stable and medically cleared for discharge, and the Physician notifies the nurse and prepares the discharge orders. However, the social worker or case manager is pending authorization for home medical devices, transportation, or home care services. I experienced this most frequently while working on the weekends. Although no harm was done to the patient, the ineffective communication between the caregivers caused a delay in discharge, sometimes for the entire weekend until offices are open on Monday. As a result, patients become dissatisfied with the service provided.
