The purpose of this assignment is to provide theoretical and empirical knowledge to support the analysis and discussion of an issue surrounding the activity Elimination. This reflection is part of an ongoing portfolio that extends to include how being involved in the planning and implementing of a care plan promotes confidence in developing professional competencies expected as a Registered Nurse according to the Australian Nursing and Midwifery Council (ANMC) and the Australian Health Practitioner Regulation Agency (AHPRA). Topic and Instructions: Identify a situation that describes an individual patient's needs around Elimination. You are encouraged to reflect upon a clinical experience that you participated in, however it is not essential to have experienced this situation first-hand, you may choose to reflect upon your experience from a particular class discussion or case study. The reflection is about what happened and any suggestions that may have improved the situation. Did the situation affect only you and the patient, or did it also affect the patient's significant others and/or colleagues? You must reflect upon how you feel the situation has impacted on your personal learning while caring for a patient. You can mention how this impacted on you and how you or others may have handled this incident differently. This requires identifying strengths or weaknesses. The specific situation will be discussed, analysed and reflected upon. You are required to utilise current evidence to support nursing interventions and/or strategies that might assist in future practice. Action-orientated (reflective) level of reflection Now what ... do I need to do in order to make things better/stop being stuck/improve my patient's care/resolve the situation/feel better/get on better/etc.? broader issues need to be considered if this action is to be successful? might be the consequences of this action? Using the nursing process, you are also required to develop a nursing care plan around this situation identifying goals and how they could be achieved. This nursing care plan must be the plan of care that you would initiate if you were looking after the patient again. If you do not believe that goals were met in your described situation, then you will need to identify how they could have been met and develop a nursing care plan accordingly. This means that the description of the situation on which you are reflecting and your proposed nursing care plan may be different. (Table below) Nursing Process Assess Plan Implement Evaluate The reflection must be presented with a brief introduction (approx. 450 words) of the patient and description of the incident providing information about influences such as culture, age, gender, ethnicity, religion or disability. Confidentiality is essential and a pseudonym must be used. It should also include: -body of discussion reflecting on the described situation (approx. 800 words) and -a completed Nursing Care Plan using the template provided-electronic version available on eLearning (approx. 800 words) -conclusion (approx. 450 words). The content and discussion must be supported with referencing to text and current journal material. An expectation is that the recommended texts for the subject (Holland et al. 2008, and Crisp & Taylor 2009) will be used as primary resources. The main areas of emphasis for the marking criteria of the assignment are your reflection of the incident, the referencing, and the style and organization of the written work.
