A 59 yo male comes to the office c/o cough, SOB, and increased sputum production. He states that for the past 2-3 days he is coughing up more sputum than usual and it's thicker. He took amoxicillin about 6 months ago for a COPD exacerbation. This is his 3rd exacerbation this year with the last one being 5 months ago. Med Hx: COPD with chronic bronchitis, diabetes, hypertension, hyperlipidemia. Social Hx: No ETOH; quit smoking 3 years ago (40 ppd history) PE: Vitals: 140/83 mm Hg; HR 80 bpm; RR 20 breaths/min; T98.8F Appearance:Appearance: A & O x 3; no acute distress; CV: RRR NL S1 and S2, no murmurs Lungs: slightly barreled chested; clear anteriorly; scattered rhonchi and expiratory wheezing posteriorly.
