CASE STUDY : Mr. Singh is an 85 year old gentleman who lives in a long term care home. He has resided there for a number of years after having a stroke which affected his left side and his speech. He requires assistance with all of his ADLs. Mr. Singh has Type 2 diabetes and takes Gliclazide 80mg BID and Metformin 1000mg BID.

Today Mr. Singh has had little appetite and refuses to eat more than a few bites at breakfast or lunch. He received all his medications as usual this morning. At 1:30pm the nurse notices Mr. Singh is pale, diaphoretic and very lethargic (responding only to touch).

1. Considering the above scenario, what do you think is going on with Mr. Singh and what caused this (describe the pathophysiology of this)?
