You have almost finished showering Mary when another carer knocks and asks for you to help transferring another resident with the lifting machine. You hand Mary the showering hose and tell her not to move you will be back in a minute.
You go and help the other carer and when you return you find Mary on the floor in the bathroom moaning.

Case Study B:
Matthew has begun urinating in unusual places. There have been several instances with one family of another resident finding him urinating in their father's room.
Staff raised these behaviours as a concern. The specific behaviors were observed and documented accordingly. Staff realised for their brainstorming that Matthew is not actually incontinent but must be having problems interpreting where or what a toilde is.
