Carl is a 52-year-old African American male currently incarcerated in prison and has been for the past 5 years. He currently is co-infected with HIV and Hepatitis - C. He is on a twice daily regimen of Combivir and Kaletra. He is reluctant to take additional medications. He has an extensive history of IV-drug engaging in speed balling with drugs of choice being heroin and cocaine. His most recent labs indicate that he has a CD-4 count of 700 and that his viral-load is undetectable. He is experiencing increased issues of irritability, agitation, insomnia, poor concentration, and passive thoughts of dying. He denies previous use of any psychotropic medication trials to treat these symptoms. His presentation is sincere, and genuine and he is wanting assistance and is providing assertions of safety. In the past week he has received word that his mother has passed away unexpectedly, and he is experiencing increased feelings of guilt and shame.
