Assignment:
There has been much discussion (especially as of late!) on how we can make healthcare accessible to all, regardless of geographical location and other barriers - what we have learned to be pervasive health. One of the related aspects to many of the barriers in these regions, in addition to lack of access, are the social determinants of health.
Digital health solutions hold the potential to have a system developed in which patients can be screened for the social determinants of health. This screening process would then help to identify which barriers to care the patient is experiencing, as well as guide a treatment and access plan based on their individual needs/circumstances.
Please read the article below to better understand this idea, and what work is currently being done.
Develop a brief plan and rationale for a system that could do just this in our area (Northern Ontario). You will be developing an infographic that includes the following components:
1. Some relevant statistics highlighting the impact of the social determinants of health on the health status of individuals (you may choose to focus on only one determinant, or more than one, depending on what the goal of your system is).
2. How will your system contribute to the idea of pervasive health - how will it help ensure the best care is provided to those who may not have regular access?
- Will it allow physicians to better understand the patient before meeting?
- Will it highlight what the most likely contributor to their lack of access is? (Education? Socioeconomic status? Race?)
- Will it help to identify where healthcare resources should be allocated to improve the health of a specific population?
- Will it use the social determinants to get ahead of negative health outcomes?
3. What is your plan of implementation for this system (specific areas/suburbs? cities? provinces?) How will it be monitored/tracked? Will it sync with EHRs?
4. Use of visuals, correct spelling/grammar, and references (in APA).
