Patient education is linked to compliance and better outcomes. Readmissions are an issue across the board in every area- all departments, which leads one to believe that there must be a common denominator. Therefore, when it comes to education, it is no longer okay to say, "I charted it and that means I did it" and pass the responsibility onto the patients. The IOM has an interesting video out there that discusses an issue in this country referred to as Health Literacy and patient outcomes. Health literacy is what spawned all the new policies you see at work. Caring that our patient does truly understand health care teaching and have the resources to follow through is very important to successful discharge plan. How can you identify if your patients are health illiterate? How will your communication strategies change once you know how health literate your patient is?
