Her medications include Amlodipine, Eliquis and Aricept. At 10:45 am today Mrs. Blum lost her balance while walking in the hallway with her cane and fell in supine position on the floor. She sustained a linear laceration on her right occipital area measuring 3 cm. with moderate bleeding controlled by pressure dressing. Mrs. Blum denied losing consciousness, mental status at baseline, PERLA, c/o localized pain on site of laceration 3/10, no limitations in ROM to all extremities. her Vitals are as follows: BP= 148/89 PR= 74/min RR= 19/min Temp= 96.8 O2 sat= 97% on room air. Mrs. Blum's MD Dr. Berman ordered to send her to St. Margaret Hospital for evaluation and possible CT scan of head.
