How to create an implementation plan on how poverty has affected black African Americans who are within the poverty line in Wilmington Delaware and have limited access to proper health that:

1. Describe how the agency will address the problem statement and list the outcome(s) to be achieved by the end of the project or program period.

2. Demonstrate the use of epidemiologic and social determinant data within the geographical location and how the organization used that data to select the focus area of the selected topic to work on over the project period. (Look at city, county or state health departments as well as the CDC.)

3. Identify program activities that will be implemented to meet the project period outcomes. What frameworks and models are being used? Provide evidence from the literature to support your approach.

4. Provide the goals and objectives of the proposed program (take from Week 3 Goals and Objectives)

5. Describe how the program activities will benefit the entire intended population.

6. Describe how the program activities are culturally appropriate for the selected target population or group. For example, it might be based on race, ethnicity, nationality, sexuality, or gender identity.
