Assignment:

You are the RN working in the Emergency Department (ED) today. You are helping with a patient who was just brought in for further assessment of her condition. You meet Toya, a 22 year old female. She came to the ED because of worsening abdominal pain over the day and half and continues to get worse. She tells you that she tried resting and changing her diet to liquids and bland foods the first day, but her pain got worse overnight. Today she has not been able to eat or drink much. She states nothing she has

tried makes her pain better. She describes to you the location of her pain was in the middle of the central abdomen but today it feels more specific to her right side, lower than her umbilicus. You note her facial expression is that of a grimace. You take her vital signs which are as follows: T 100.5 oral, HR 110 upon palpation, RR 17 upon observation, BP 122/84 taken manually in RUE, pulse oximeter is 99% on room air, and she is rates her pain 8 out of 10 on a numeric scale. She lies down once you are done and you note the grimace on her face is now less pronounced. She tells you she is beginning to feel a little nauseous now too.
