You are the HR manager for a warehouse. Warehouse employees use forklifts and special machines to tape and wrap packages on a daily basis. Naturally, these machines sometimes break down and require repair. You have proper safety procedures in place for repairing equipment along with a variety of requirements for employees to wear personal protective equipment. One day, a forklift breaks down and a newer employee is tasked by a manager to repair it. This involves using a machine to lift the forklift off the ground and attaching a crane system to secure it. However, the employee did not properly secure the crane system and during he repair the forklift fell on the employee. The employee's arm was broken in the incident and he was hospitalized for a few days with other injuries, though eventually recovered fully. You conduct an investigation and discover the employee was properly trained but simply failed to secure the strap properly as all other employees seem to have done in the past.

a. How should this incident be recorded and reported to OSHA?

b. Given that the employee was properly trained, what could have caused this accident? What can you do from an HR perspective to help ensure accidents like this do not happen in the future?
