HPI: The patient is a 65-year-old female with a psychiatric history of generalized anxiety disorder (GAD), Major depressive disorder (MDD), and attention deficit hypersensitive disorder (ADHD) here in the clinic today for a follow-up visit to review her medications. Her current medications are Prozac 40mg daily and Adderall XR 15 mg in the morning. The patient reported she is recovering from an ear infection. The patient also stated today is her son's birthday who passed away and is very stressful. Patient is requesting an additional Adderall to help with motivation and to get her off the couch. The patient reported that sleep is not the best. She reported her appetite as adequate and added that she has been exercising and watching her diet but still has trouble losing weight.
