Question 1-- In a lifting system failure, (or any other type of incident) do you agree with the supposition made by H. W. Heinrich that 88% of all incidents have human error as a root cause (Ranjan, 2018)? Why, or why not?
If the Occupational Safety and Health Administration (OSHA) were to write a violation, would the violation go to the company or the individual who was in charge of the lift? As you respond to classmates, discuss whether or not you agree with OSHA's violation policy.

Question 2--If you have new housing construction going on in your area, do a "drive by". Look at the employees working at a raised elevation. (Just look - without engaging with the workers or anyone else on site), Or, if there is no new construction, are any of your neighbors doing work on their roofs?
How many of the workers are wearing and using fall protection? Do an informal survey, and let us know how many, if any, are using fall protection.
Discuss one such worksite, either good or bad, and why you chose it.
