Case Scenario

Mrs Olivia West, 64 years old, is hospitalised for a right knee replacement. In the course of a routine physical examination, the resident discovers a nodule in the left side of her thyroid gland. Careful questioning reveals that, for many years, Mrs West tended to be warm when others were cold. The resident also notes that the client has quite fine hair and soft skin. In addition, the client recently lost about 9.6 kg and now appears thin and frail. All of these are indications of a hyperactive thyroid gland. Based on the history and physical examination, thyroid function studies are ordered. The thyroid scan following a tracer dose of I 131 confirms the location of a thyroid nodule associated with thyroid hypersecretion. The client is started on propylthiouracil (PTU) 100 mg bid PO. She is instructed to call her healthcare provider if she experiences a skin rash or sore throat.
