Mary is a 65yr old female with a past medical history significant for Multiple Myeloma (treated with Cyclophosphamide/Bortezomib/Dexamethasone), currently under observation, hypertension, chronic kidney disease stage IIIb secondary to Multiple Myeloma, hyperlipidemia, migraines, osteoarthritis, and hypothyroidism. She has been following with her PCP on a routine basis. Recently, she came to the office with reports of dizziness when standing and feeling fatigued at times. She has not fallen or lost consciousness. No recent illnesses, chest pain, nausea, vomiting, or diarrhea. Shares that she does get short of breath with climbing flights of stairs or walking long distances. No changes in bowel or bladder habits. Shares that she eats a general diet without restrictions.
