M.P, a 70-year-old retired postman, has a long history of vascular disease. She was diagnosed with type 2 diabetes 20 years ago and had a four-vessel coronary artery bypass graft 2 years ago after myocardial infarction. She also has a history of hypertension, hyperlipidemia, heart failure, and hypothyroidism. She has been experiencing marked pain in her left foot and leg at rest, with numbness and tingling in her toes and feet for the past few months. She has had pain for several years in both her buttocks and calves when she walks, and this was the main reason she retired from the postal service. she has recently become a widow, she is very depressed. her children do not live near her and for this reason, she is now in-home care.

Her current medications include furosemide (Lasix), metolazone (Zaroxolyn), enalapril (Vasotec), simvastatin (Zocor), clopidogrel (Plavix), levothyroxine (Synthroid), metformin (Glucophage), and Sertraline.
