HISTORY OF PRESENT ILLNESS: is a with pre B ALL on interim maintenance therapy brought in by his parents for intermittent fever to 102 x4 days. is also complaining of leg pain for the past few days, and today has refused to walk. Last night was able to jump around after his fever broke, family feels that his joint pains are related to the fevers, and has complained about every joint including elbows, ankles, knees. Family has trialed tylenol and oxycodone for pain with no improvement. has had some decrease in appetite. No sick contacts at home. +cough which has been present for about 2 weeks, and has been productive, causing some post-tussive emesis after swallowing mucous. In regards to his port site, there has always been hyperpigmentation around the access areas, but in the past 2 weeks it has spread to the axilla, and has complained of pain. Methotrexate and Vincristine given in clinic on. Started on zithromax for 10 days of cough +crackles/coarseness noted on exam at that time.
