Item 1 of 1 A nurse is caring for a 78-year-old client who is recovering from hip replacement surgery. Nurse's Notes Vital Signs 4/28 0900 Review of the plan of care indicates that this client will start ambulating using an assistive device. Provider made an order for a consult with the physical therapy department. Last documented report shows that client tolerated physical activity in preparation for ambulation, except drop in blood pressure when attempting an upright position. Nurse observes that client sits up and assumes dangling. A declining trend in blood pressure from supine to dangling was noted; client denies dizziness or lightheadedness when sitting and dangling. Tolerated upper arm strengthening and isometric exercises. Client reports feeling anxious about ambulating after surgery.
