Question

Cory is a 62 year old patient who received a heart transplant almost a year ago. Before the surgery, other than his heart failure, Cory was relatively healthy and was considered a good candidate for transplant. Unfortunately, his recovery did not go as planned, and Cory has spent nearly one year in the ICU. Complications Cory experienced after surgery include multiple instances of respiratory failure requiring intubation, which led to a tracheotomy and ventilator dependency. Cory also experienced kidney failure and is reliant upon dialysis. Recurrent pancreatitis has been another complication post-transplant, eventually requiring a feeding tube. Over the past several months, Cory has expressed to nurses that because of the burdens he experiences daily, he does not want to continue with all of the medical therapies he is dependent on. A nurse requested an ethics consult, asking for help supporting Cory's end of life wishes. While some family and care team members were supportive of Cory's wishes at this point, others were concerned that he had lost hope, wasn't thinking clearly, and was not truly appreciating the consequences of his choice. What can ethics committees do to help support cory and the health care team.
