Nurses in particular are in a position to be patient advocates and coordinate with other members of health care teams to ensure continuity of care."( (Regis College, 2023)" The role of the nurse is to ensure that all team members are caught up on the needs of the patient. The nurse spends the most amount of time with the patient and in turn is responsible for keeping the standard of care high. One way the nurse does this is through SBAR reports. SBAR stands for Situation, Background, Assessment and Recommendation. The nurse uses the SBAR to communicate and share patient information among the entire team caring for the patient. Ensuring that a clear and complete SBAR is shared is critical for a high standard of continuity of care. The SBAR enhances the communication by standardizing what needs to be conveyed. When there is a framework to work within, it is harder for the nurse to forget important information.
