Formulate a psychiatric SOAP note and include psychiatric diagnoses, differential diagnosis according to the DSM-5, and treatment plan: 50 y/o female was brought into a psychiatric by the Police clinic for a virtual psychiatric evaluation after being found at a bridge stating the voices were telling her to jump. She reported hopelessness, helplessness, decreased energy, trouble sleeping, homelessness, and no interest in any activities, which have worsened since her husband left her and took their two children. She has never seen them for several years. She was alert and oriented x 4. Cognition appeared grossly intact and appropriate attention span, concentration, and knowledge. She reported a decreased appetite and has lost 12 pounds in one month. She stated she was tired and did not want to live anymore. She reported suicidal ideations and heard voices several times telling her to jump off the bridge. She reported the voices saying, "Today is the day to jump." She reported voices from when she was 7 years old from Margorie, which she liked because this voice kept her company. She reported she uses alcohol and marijuana whenever she can get it. She smokes cigarettes daily. She was arrested once for vagrancy. She was prescribed Mirtazapine 15 mg po at bedtime and referred to inpatient psychiatric hospitalization for further management, which she agreed voluntarily.
