Question

A 75-year-old patient is seen in a follow-up outpatient appointment. During the assessment, the patient is noted to have their arms crossed and is making poor eye contact, which is not typical of their usual engagement. The patient tells the nurse practitioner they are having difficulty with their spouse. On further discussion, the patient reports that their worry has worsened over the past year, and the constant restlessness due to worry, fatigue, poor sleep, and irritability have caused their spouse to move to the guest bedroom. The spouse would like the patient to consider medication. The patient responds to the nurse practitioner's questions about medication by saying, "It's fine" and "It's not a problem. Pills won't change how I am," and they scowl. What is the nurse practitioner's best response to this patient? I am not sure what you want from me today since I have told you medication will help, but you continue to say no It sounds like you don't want to be here today, and I'm wondering if we should reschedule for another time Shutting down and not talking about the problem is not going to help make things better between you and your spouse You are saying things are fine, but I am noticing your expression changed when you said that, and I'm curious what you think about that.
