Services include service coordination and collaboration, assessment, individualized treatment planning that can include medication management, individual therapy, family and group therapy (as appropriate), safety and prevention planning, caregiver education and support. The DELTA program provides additional community training to caregivers, individuals, and community members, as well as consultation and resource identification to other service providers.
