Emily, thank you for sharing some about your practice problem to help reduce the HbA1c of these diabetic patients that are seen in your practice. We have seen only too often, the challenging outcomes these patients have when they are not focused on controlling their disease. Sadly, this tends to be a significant issue for many diabetics. I agree that it takes the entire team to work together to align their goals to help develop and implement a plan that will aid in this. Does your office employ a dietician or is this something that gets referred out? Does your team include social workers as well? Many of these patients often lack additional resources that may be needed in order for them to accomplish this goal.
