Meet Meredith Grey, an 83-year-old female diagnosed with Diabetes, Hyperlipidemia, Hypertension, and Major Depression. Recently, she visited the ED due to bruised ribs after falling in the shower. Meredith lives alone since her husband passed away earlier this year. Her son lives nearby and checks on her regularly, usually after work or on the weekends. Although Meredith enjoys reading, she has difficulties going to the library frequently because she doesn't drive and relies on her son for transportation. Upon reviewing Meredith's chart in the EMR/EHR, ?you notice that she hasn't seen her PCP in the last 5 ?months. She has canceled or no-showed for 3 ?appointments, including the one scheduled after her ED visit. You also observe that she's been non-compliant with regularly checking her blood pressure, as directed by her PCP as part of the RPM program for Hypertension management. When you speak with Mrs. ?Grey, she opens up about feeling very alone since her husband's passing and her frustration with her reduced independence. She mentions that her husband used to handle most of the grocery shopping and cooking. Since his passing, her diet mainly consists of canned items or frozen meals. Meredith asserts that she takes her medications as instructed but reveals that she misses appointments because she forgets to inform her son about it.
