Question

Emma is 86-year-old with a history of hypertension, hypercholesterolemia and diabetes who presents to the Emergency Department after 3 days of fatigue and shortness of breath with activity. Emma is a retired engineer and lives independently at home with her husband and dog. She states, "I participate in yoga, run 3-5 km once a week, and drinks 7-9 glasses of wine a week." She denies chest pain but complains of epigastric pain. She reports her epigastric pain has gotten worse and is now radiating to her jaw and is constant. Emma appears anxious and is repeatedly asking foe her husband upon arrival at the hospital. Emma is on a stretcher grasping her jaw and saying "i feel like I am going to die" Vital signs are as follows: T 36.7, HR 118 (regular), RR 24 (regular), BP 159/62, O2 99% on room air, Pain 8/10, Height and weight 170cm, and 210lbs 1.
