Question

Interpersonal and Extrapersonal Factors. Dianne, a 82 - year - old white female who lives alone, presents to the family practice with a chief complaint of "extreme fatigue and shortness of breath for 2 days." Last week he had lunch with his daughter, granddaughter, and great-grandchildren, who had an upper respiratory infection. She developed a cold last week that was treated with Mucinex DM. She did not seek health care, since the medication was helpful; and she thought that it was resolved. She admits to increasing fatigue and found it difficult to "get started with her usual routine this morning." The fatigue initially started with Shortness of breath; she has been unable to take her daily short walk and did not want to get out of bed this morning. Yesterday, she forgot a scheduled appointment with her dentist and forgot to take her medications. She denies changes in vision or hearing. Dianne denies headaches and myalgias. She denies fever but admits to "some" shortness of breath with exertion. Dianne denies cough, chest pain, or palpitations, but admits to feeling congested and having tightness in his chest at intervals, especially with exertion. She denies rhinorrhea, dysphagia, otalgia, sputum production, fever, or chills.
