"Therapy is not sure what is going on with pt. they did express interest in wanting the power chair but when Power chair was brought out to facility for pt. is refusing to use it. Pt is reported by therapy that he might be feeling like he is not getting the help he needs. Because right after therapy session he is going to the nurse's station and talks in Somali language to the nurses. Pt might be saying to therapy what he thinks they want to hear. He told therapy staff that they abandoned him. He continues to have high expectation and demands a lot on therapy staff and nursing staff. ACP to find out what are the 3 things he wants to work with going forward. Interpretation services during therapy and ACP sessions were suggested to get his baseline at least one time. Pt continues to work on Transfer /walking and parallel walking. A while back pt was updated to 1 assist and demands to use EZ stand with transfer and incontinence care."
