In the situation of Mildred had been experiencing altered mental status, which has been affecting her daily life. It is our job as their nurse to ensure we are sending Mildred and her son home with the correct care they need to keep her safe and so that her son will not be overwhelmed because of the lack of family help. The first step that I would take is going over medication and possibly checking blood work to ensure that her symptoms are not due to a medication interaction or adverse effects due to long-term medicines. I would then ask about their economic status and what kind of help Mildred insurance offers. In most cases, older people receive Medicaid or Medicare, which can provide an in-home caregiver who can help Mildred with daily tasks. I would also assess and ask the son more questions to help coordinate help and see if he thinks that just a part-time caregiver would be enough.
