Paul Jenkins, a 19 year old student, has experienced severe thermal burns from a house fire in his bedroom. Paul was asleep at night when a spark from a candle started a fire, leaving him trapped in his bedroom. By the time the fire department arrived, he had suffered severe burns. He was transported via ambulance to a regional hospital urgent care centre with a non rebreather mask applied with 15L of O2. In the emergency room his burns were evaluated and he was found to have primarily deep dermal partial thickness burns over the entire anterior chest and his whole left arm. He weighs 75 kg and has no significant past medical history. On assessment: A- Able to talk in full sentences- some hoarseness of his voice B- Respiratory Rate 28 breaths/min, O2 saturation level 93%, bilateral air entry into lung fields. C- Blood pressure 90/60 mmHg, pulse rate 110 beats/min- regular D- Alert and orientated, pain score 8/10 across chest and 10/10 in L) arm. Nil other pain sites reported. E- Temperature 35.4 °C, Facial oedema present, singed nasal hairs. Body exposed nil other- burn sites visible. peripheral oedema present on all 4 limbs. What is the TBSA of the burn injury? Using the modified Parkland formula, calculate the fluid resuscitation requirements for this patient. What adjustments need to be made to his oxygen therapy? What assessment findings indicate that Paul has sustained an inhalation injury? What other assessments are necessary, given that Paul's entire left arm is.
