PARAPHRASE Within the healthcare sector, the discharge process is important as it ensures continuity of care and improving patient outcomes. However, in most cases, confusion always arise when it comes to medication regimens post-discharge that can results to poor adherence, hospital readmission and adverse events (Jerath et al., 2020). In this paper, the issue of ineffective discharge communication is discussed particular in regards to medication instructions as a practice clinical problem in the healthcare sector. The purpose of this paper is to identify and propose a solution for the practice problem in relation to discharge processes, by focusing on how to improve patient understanding of medication regimens after leaving the hospital. There are three main sections that this paper discusses including the specific clinical problem in regards to discharge processes in a healthcare setting, an analysis of the practice gap based on the needs assessment of the organization, and finally a formulated practice question guide for the proposed practice change project.
