CommonSpirit Health's 2021 NAHSE case study, "Value-Based Care: The Next Era in Healthcare," emphasizes both the imperative for and urgency of changing the U.S. model for healthcare delivery. Based on the intricate dynamics of managing chronic disease and health disparities, not only does the case highlight the cost burden that chronic illness imposes on healthcare, but it also addresses and faces the inequities inherent to existing care models especially for those who are most underserved. As a result, moving away from a volume- and toward a value-driven model becomes not only an economic must but an ethical necessity. Most striking here is the powerful connection between determinants of health and health results and with particular reference to minority communities. In Phoenix, Arizona, where conditions like diabetes, obesity, and heart disease disproportionately affect communities of color, the shortcomings of a fee-for-service model stand out apparent. Prioritizing care volume over care quality, the legacy model not only sustains inequities, but it is unable to effectively prevent and manage high-cost, preventable conditions. CommonSpirit's approach to incorporating equity into efforts at population health and forming alliances with community-based providers is an exciting model for addressing these long-sustained disparities.
