Molly Young is a 9- year-old female that presents to the emergency department after the school nurse called her mother. Molly has a history of moderate persistent asthma. She was diagnosed at age six and had two prior hospitalizations for asthma, with one admission to the pediatric intensive care unit. She has been at 70 percent of her personal best measurement for her peak expiratory flow (PEF) meter reading which did not improve with the use of albuterol metered dose inhaler (MDI). Molly started complaining of chest tightness and feeling "winded" after recess today. Molly is alert in the emergency department, sitting upright and speaking in short sentences with an O2 Sat of 91% on RA. She complains of increased chest tightness and uses her accessory chest muscles to breathe. She has coarse expiratory wheezes throughout both lung fields with decreased breath sounds at the left base. Moderate intercostal and substernal retractions are noted. Molly appears anxious.
