Question

You make some great points with EHRs. EHR's reduce medical errors and leads to an improved patient safety, it also promotes communication and collaboration among healthcare teams. I feel that it keeps everyone on the same page when it comes to collaboration between the care team. Although, I do read documentation daily and sometimes the attending hardly ever reads their consults notes because they do a lot of contradicting when it comes to the treatment plans. I then have to send them a query for documentation clarification because their plan of care differs from the consultants and leads to conflicting diagnoses..
