You are the coding supervisor at Vale Community General Hospital. A HIM student, Rachel, is working with you today. She is looking at some charts and has asked you how to determine if the documentation supports the codes. You decide to walk through the charts with her and explain each one. The principal diagnoses are shown in the following table. What would you tell Rachel about each? Include the symptoms, method of diagnosis, treatment, etc. that she should look for. For this assignment you aren't coding the diagnoses. You aren't furnishing principal diagnosis. What you're doing is looking at the diagnoses and will be writing sort of what would be in your head and what you'd think about and check in such scenarios what the clinical workup would be/show. It's a good exercise for everything that leads up to coding and chart analysis. Look at the example below given with pneumonia - you should be writing something similarly for each diagnosis listed. Research as needed. Similar to the pneumonia example below, for each of the others, like sepsis, what sort of symptoms or clinical indicators (like lab results and types of stuff the doctor would show and do) would you.
