You are part of the interprofessional team discussing the poor pain control of Mrs. L., who is in the end stages of aggressive breast cancer with metastases to her pancreas and liver. She is only 48 years old, married with a supportive husband, and two young, teen-age children. On any pain scale provided and in verbal discussion with her, she is scoring 9-10/10 "around the clock". Mrs. L. is begging all the team members for more analgesia to "put her out of her misery", but the physician leading the team feels that if she receives enough medication to alleviate most of her pain and suffering, it may render her unconscious and thus she cannot interact with her family.
