Assignment:
CH is an active 64-year-old woman. She volunteers several days a week at a local senior center. She has recently developed increasing abdominal pain, accompanied by a 25-lb weight loss. An MRI scan showed a large mass in the fundal area of the stomach. A subtotal gastrectomy was performed. She is now post-op day #1.
Subjective data: States her abdominal pain is 8/10.
Objective data: Vital Signs: 155/74 mm Hg, HR = 104/min, RR = 20/min, SpO2 = 93% on room air, T = 100F
Oriented to person, place, and time. Diminished breath sounds with crackles in bilateral lower lobes. Skin warm and dry. Nasogastric tube to low continuous wall suction draining brownish-green drainage. Intravenous fluid infusing at 100 mL/hr. Bowel sounds absent all four quadrants. Abdomen tender and slightly distended. Abdominal wound clean and dry with intact dressing.
