I have to give a primary diagnosis with two differential diagnosis for scenario. This is for my FNP adult health class that requires us to analyze and come up with the diagnosis:

A 54 year- old male comes to the office for an exam. He has not had any care for the past 10 years due to work and not insured. The patient is found to have elevated blood pressure of 150/86. Physical exam unremarkable. BMI of 27-overweight. Has gained 20 pounds in 5 years. Family history: mom has high cholesterol and diabetes, father died of heart attack at 63 yeas old. patient is a taxi driver, has financial stress, sedentary life style, eats fast food when he works. two reasons were performed on the same visit on both arms 159/86 in the right arm and 151/82 on the left arm. Patient is sent home to log his blood pressure for two weeks and follow up. What diagnosis and two differential diagnosis would you give to this patient?
