Differential diagnosis for 60-year-old female pt is here today for heart palpitations. The patient is alone in the clinic today. Patient states she has been feeling like her heart has been feeling funny and it has been occurring off/on for approximately 1-2 months. Patient reports the palpitations are typically felt in the chest and throat area and last about 5 minutes. She describes the palpitations as a racing heart along with a lump in her throat. Patient reports that they are triggered by stressful events and sometimes relieved by deep breathing. She reports they occur 3-4 times a week. Her last palpitation with yesterday while she was out at the grocery store. Patient reports she went to the ED on April 27th. They said her heart was good but the gave her metoprolol tartrate 25mg to take 1/2 a tab BID PRN for palpitations. She has no complaints of palpitations or dizziness today. She denies headaches, n/v. History of thyroid disease.
